[Remote] Care Coordinator (Home Care Operations)
Note: The job is a remote job and is open to candidates in USA. Tandem Care is hiring a Care Coordinator who thrives in fast-paced, high-volume home care environments. This role involves being the primary point of contact for clients, conducting care assessments, coaching caregivers, and ensuring care plans are effectively executed.
Responsibilities
- Serve as the primary point of contact for assigned clients and families
- Conduct onboarding calls and care assessments to establish initial care plans
- Review caregiver visit notes and feedback to identify care issues, gaps, or changes
- Coach and support caregivers on client-specific care needs and expectations
- Address client concerns and resolve issues related to care quality or consistency
- Proactively check in with clients to ensure satisfaction and adjust care as needed
- Collaborate closely with Scheduling, Staffing, and Operations teams to ensure care plans are executable
- Document all care plan updates, communications, and decisions clearly and consistently
Skills
- 2+ years experience in a care coordination, client services, or care management role within a licensed home care agency
- Direct experience working with caregivers (coaching, problem-solving, or care plan support — not just intake)
- Comfortable working remotely in a fast-paced environment
- Strong communication skills across phone, email, and internal systems
- Bachelor's degree preferred, but not required
