Nurse Practitioner - Post-Acute Care / PM&R

Practice Info

  • Provide focused care in nursing facilities
  • Oversee daily patient interactions through timely micro visits that support focused assessments to assess recovery, manage pain, wounds, and functional decline
  • Collaborate with physicians, therapists, and facility staff on individualized recovery plans
  • Monitor readmission risk and help close gaps in post-acute care

Responsibilities

  • Round in assigned facilities on a consistent weekday daytime schedule
  • Oversee daily patient interactions through timely micro visits that support focused assessments to assess recovery, manage pain, wounds, and functional decline
  • Collaborate with physicians, therapists, and facility staff on individualized recovery plans
  • Use Puzzle's EHR to complete documentation (often remotely from home)
  • Monitor readmission risk and help close gaps in post-acute care
  • Maintain compliance with post-acute and PM&R-related regulations

Shift & Schedule

Requirements

  • Master's degree in Nursing and current certification as a Nurse Practitioner (NP) from an accredited program
  • Active, unrestricted NP license in the relevant state(s) of employment, with prescriptive authority
  • Minimum of 2-3 years of experience in PM&R, geriatrics, post-acute care, or a related field; experience in nursing facilities strongly preferred
  • Strong clinical skills in assessing and managing barriers to recovery, including rehabilitation, pain, and chronic conditions
  • Excellent communication and time-management abilities to handle brief, efficient patient encounters
  • Proficiency with EHR systems and remote documentation
  • Ability to work independently with a high level of autonomy and consistency
