Schedule: Monday-Friday 8 AM-5 PM
NO CALL!
Service Area: Metro Wichita area (covers a 50-mile radius and the 5-county metro area)
Requirements
Must have at least 1 year of Home Health experience within the last 3 years as an RN.
HCHB and Oasis charting experience is also required.
Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine individualized care plans.
- Administer medications via IV infusion, injections, and other routes following aseptic techniques and safety protocols.
- Manage complex wound care procedures such as suturing and ostomy care while ensuring infection control standards are maintained.
- Perform specialized treatments including botulinum toxin injections, dialysis management, and gastrostomy tube feedings.
- Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic, Cerner, Athenahealth, or eClinicalWorks for accurate documentation of patient care activities.
- Provide education on disease management, medication administration, and health coaching tailored to patients’ needs across diverse settings like assisted living, nursing homes, or home care environments.
- Collaborate with physicians and interdisciplinary teams to develop discharge planning strategies and coordinate long-term or post-acute care services.
Qualifications
- Valid RN license with current state registration; additional certifications in Basic Life Support (BLS) required.
- Proven experience in home health nursing or related fields such as hospital medicine, critical care, ICU, or emergency medicine.
- Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth; experience with ICD coding (ICD-9/ICD-10) is preferred.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology essential for accurate patient assessment and documentation.
- Experience working with individuals with disabilities or developmental delays; knowledge of hospice & palliative medicine is advantageous.
- Ability to perform complex procedures such as IV insertion, phlebotomy, catheterization, and sterile processing techniques confidently.
